Breast Augmentation Surgery

Breast augmentation can be performed using two main approaches. Depending on the patient’s goals, anatomy, and the surgeon’s assessment, either technique may be used alone or both may be combined during the same procedure, known as the Hybrid Technique.

1. Breast Implant Augmentation

Breast volume and shape are enhanced using implants selected to suit the patient’s anatomy and desired outcome.

2. Fat Transfer Breast Augmentation

This procedure uses the patient’s own fat cells to enhance breast volume. One advantage is that it can provide a natural-looking and natural-feeling result.

However, the patient must have enough donor fat to achieve the desired size, and the amount of transferred fat that remains may vary between individuals.

Types of Breast Implants

Classification by Shape

Teardrop-Shaped Implant

A teardrop-shaped implant may be suitable for individuals with a slim body frame who prefer a natural-looking breast contour with less fullness in the upper breast.

For this type of implant, an incision beneath the breast fold may be considered to help the surgeon position the implant accurately.

Round Implant

Round implants are commonly used because they can provide greater fullness in the upper breast and are available in various sizes to suit the patient’s desired appearance.

Ergonomix Implant

The Ergonomix implant is designed to adapt to body position. It may appear more teardrop-shaped when the patient is standing or sitting and more rounded when lying down.

Classification by Implant Surface

Smooth Surface

Smooth implants have a relatively thin outer shell and may feel softer. They can move more freely within the implant pocket, although this mobility may increase the possibility of displacement in some cases.

Textured Surface

Textured implants have a thicker and rougher surface designed to adhere more closely to the surrounding tissue. They may reduce implant movement, although rippling or visible surface irregularities may occur in some patients.

Nano-Surface or Nanotechnology Implant

Nano-surface implants are newer-generation implants with a fine surface texture. They are designed to combine softness, stability, and a reduced likelihood of excessive scar-tissue formation, depending on the implant model and individual response.

Implant Placement

Under the Chest Muscle — Submuscular Placement

This placement may be suitable for individuals with limited natural breast tissue. Positioning the implant beneath the muscle can help conceal the implant edges and reduce the visibility of rippling.

Above the Chest Muscle — Subglandular Placement

This placement may be suitable for individuals with sufficient natural breast tissue to cover the implant. It may provide a natural contour and may involve a shorter initial recovery period.

The most appropriate implant type, surface, size, and placement should be selected according to the patient’s anatomy, preferences, and the surgeon’s assessment.

Preparation Before Surgery

  1. Prepare your body by exercising regularly, getting adequate rest, and maintaining good general health.
  2. Avoid anticoagulants, steroids, pain-relieving medicines, anti-inflammatory medicines, vitamins, and dietary supplements for at least two weeks before surgery, as advised by the surgeon. Do not stop prescribed medication without consulting the prescribing doctor.
  3. Inform the surgeon of all underlying medical conditions, regularly used medicines, previous operations, medication allergies, and dietary supplements.
  4. Stop smoking and avoid alcoholic beverages for at least two weeks before surgery.
  5. Do not eat or drink for at least eight hours before surgery, or follow the fasting instructions provided by the anaesthetic team.
  6. Remove nail polish and artificial nails before surgery, as the anaesthesiologist may need to measure blood oxygen levels through the fingertip.
  7. Should you develop an illness, such as fever, cough, sore throat, diarrhoea, an infected wound, red eyes, or eye pain, inform the nursing team at least two days before surgery.
  8. Patients with risk factors for breast cancer may be advised to undergo breast screening, such as a mammogram, before surgery.
  9. Avoid scheduling surgery close to or during menstruation when possible. Consult the doctor should menstrual-cycle planning be required.
  10. On the day of surgery, do not wear contact lenses, dental retainers, dentures, jewellery, or bring valuable belongings.
  11. Plan to recover at home for at least five to seven days to reduce discomfort and support wound healing before returning to normal work.

Care After Returning Home

  1. Sleep on your back with your upper body elevated approximately 45 degrees for at least two to four weeks.
  2. During the first two to three days, clean only the lower part of the body and use a sponge bath for the upper body. Showering may generally begin on the fourth day, provided that a waterproof dressing is applied to prevent the wound from getting wet.
  3. During the first two weeks, wear the breast-support bandage continuously. A support bra should be worn at all times, including while sleeping, for approximately three to six months or until the breasts have settled, according to the surgeon’s instructions.
  4. Breast massage may begin carefully after the stitches have been removed, usually at approximately two weeks. Massage for around five to ten minutes twice daily, but only when recommended by the surgeon, as massage is not suitable for every implant type or surgical technique.
  5. Avoid sexual activity during the first two weeks, as excessive movement or pressure may cause the surgical wound to separate.
  6. Avoid swimming and soaking in a bathtub for at least eight to twelve weeks after surgery, or until the surgeon confirms that the wound has healed sufficiently.
  7. Avoid lifting heavy objects, driving, running, and jumping for at least eight to twelve weeks, as excessive movement may affect the implant position.
  8. Avoid underwired bras, rigid-framed bras, and adhesive wing bras for approximately one year, or according to the surgeon’s advice. A support bra is generally recommended to reduce pressure on the implants and help maintain their position.
  9. Wear comfortable, loose-fitting clothing, such as a front-buttoned shirt, as the chest may feel sore during the early recovery period.

Surgical Wound Care

  1. Take the prescribed anti-inflammatory medicine, antibiotics, and medication for swelling as directed by the doctor. Pain medication may be taken every four to six hours when required. Follow the doctor’s instructions carefully.
  2. Approximately seven days after surgery, the doctor will arrange a follow-up appointment to examine the wound and remove the stitches when appropriate.
  3. Avoid exposing the wound directly to water during approximately the first seven days, as moisture may increase the risk of inflammation or infection.
  4. Do not press, pick, scratch, or rub the surgical wound, as this may injure the tissue and increase the risk of infection.
  5. Do not apply scar-reducing creams or other products until the wound has completely healed, unless advised otherwise by the doctor.

Food and Beverage Recommendations

  1. Avoid dietary supplements and vitamins for approximately two weeks.
  2. During the first one to two weeks, avoid foods that are difficult to digest, including very spicy foods and foods that may cause digestive discomfort. Choose soft, easily digested foods such as soup or rice porridge.
  3. Avoid fermented or pickled foods for approximately one month, as advised by the medical team.
  4. Avoid alcoholic beverages for approximately two weeks and smoking for approximately two months to support wound healing and reduce the risk of inflammation or infection.
  5. Choose foods rich in protein and vitamin C, such as eggs, milk, meat, oranges, and lemon juice, to support the healing process.

Expected Results

The breasts may take approximately three to six months to settle into their final shape. Recovery and results vary depending on the surgical technique, implant type, individual anatomy, and healing response.

Patients should follow the personalised instructions provided by their surgeon and contact the medical team promptly should they experience unusual symptoms, such as severe pain, increasing redness, marked swelling, fever, unusual discharge, shortness of breath, or sudden changes in breast shape.
